Description

Purchase and delivery of ultrasound equipment for the needs of Radom Specialist Hospital realized within the framework of the task entitled "Hospital equipment including reconstruction of rooms and equipment of the Central Sterilization Room and purchase of medical equipment, devices and apparatus for Outpatient Specialist Care together with reconstruction of outpatient clinics in Radom Specialist Hospital named after Dr Tytus Chałubiński".
Heart echocardiographic examination apparatus (ultrasonograph) for the needs of the Outpatient Clinic for Children and Adolescents of Radom Specialist Hospitalcd. from section VI.3:6.5.1. List of statements or documents submitted by the contractor in the proceedings at the request of the ordering party in order to confirm the circumstances referred to in art. 25 sec. 1 item 3 of the PPL Act:a) information from the National Criminal Register to the extent specified in art. 24 sec. a) information from the National Criminal Register within the scope specified in art. 24 par. 1 item 13, 14 and 21 of the PPL Act, issued not earlier than 6 months before the lapse of the deadline for submission of tenders; b) a statement of the economic operator on the lack of a final court verdict or a final administrative decision on arrears in payment of taxes, fees or social or health insurance contributions, or - in the event of such a verdict or decision - documents confirming payment of these receivables together with possible interest or fines, or conclusion of a binding agreement on payment of these receivables;c) a declaration by the economic operator that there has been no ruling against him/her as a precautionary measure of prohibition of competition for public contracts; d) an extract from the relevant register or from the Central Register and Information on Business Activity, where separate provisions require entry in the register or register to confirm that there are no grounds for exclusion pursuant to Article 2. e) if the economic operator has its seat or place of residence outside the territory of the Republic of Poland, instead of the documents referred to in:1) letter a - the economic operator shall submit information from a relevant register or, if there is no such register, another equivalent document issued by a competent judicial or administrative authority of the country in which the economic operator has its seat or place of residence or the person to whom the information or document refers, within the scope specified in art. 24(1). 1 points 13, 14 and 21 of the PPL Act;2) letter d - shall submit a document or documents issued in the country where the economic operator has its seat or place of residence confirming that no liquidation has been opened or no declaration of bankruptcy has been declared;3) the documents referred to in points 1 and 2 should be issued not earlier than 6 months before the deadline for submitting tenders;4) if in the country where the economic operator has its seat or place of residence or the person to whom the document relates is not issued, the documents referred to in point 1 shall be replaced by a document containing, as appropriate, a declaration by the economic operator, indicating the person or persons authorised to represent him or a declaration by the person to whom the document was to apply, made before a notary public or before a judicial or administrative authority or a professional or economic self-government body competent in respect of the economic operator's seat or place of residence;5) the economic operator having a seat on the territory of the Republic of Poland, with regard to a person having a place of residence outside the territory of the Republic of Poland, to which the document indicated in item a relates, shall submit the document referred to in item 1 to the extent specified in Article 2(1). 24 paragraph 1 points 14 and 21 of the PPL Act. If in the country of residence of the person to whom the document was to refer, such documents are not issued, they shall be replaced by a document containing a declaration of that person made before a notary public or before a judicial, administrative or professional or economic self-government body competent for the place of residence of that person.
