The subject matter of the contract is supplying the Bielsko-Biała Regional Hospital at al. Armii Krajowej 101:1) lenses, viscoelastic fluids, cataract and posterior vitrectomy surgery kits together with the loan of eye surgery equipment - part 1;2) additional accessories for cataract surgery - part 2;3) kits for complicated glaucoma surgery - part 3;4) additional accessories for posterior vitrectomy surgery - part 4-10;According to quantity and type list included in the Attachment No 2 to the Terms of Reference, hereinafter referred to as "SWZ", meeting quality parameters determined in the Attachment No 2 to SWZ, under terms and conditions determined in draft contract provisions constituting Attachment No 4 to SWZ.2The offered goods must be new and unused (except for lending apparatus for eye surgery).3. delivery of the subject matter of the contract together with transport and unloading will be carried out at the expense and risk of the Contractor.
The subject matter of the contract is to supply the Provincial Hospital in Bielsko-Biała at Al. Armii Krajowej 101: 1) Lenses, viscoelastic fluids, sets for cataract surgery and posterior vitrectomy together with lending apparatus for eye surgery - part 1; According to the quantity and type list specified in Appendix No. 2 to the Terms of Reference, hereinafter referred to as "SWZ", meeting the quality parameters defined in Appendix No. 2 to SWZ, under the conditions defined in the draft contract constituting Appendix No. 4 to SWZ.2The offered goods have to be new and unused (except for lending apparatus for eye surgery).3 The subject of the contract together with transport and unloading will be delivered at the expense and risk of the Contractor.4.For the period of performance of the contract, the Contractor shall be obliged, as part of the price of the object of procurement in Part 1, to provide, together with the first delivery of lenses, nine implant sets for reusable use consisting of an injector and tweezers for lens implantation specified in Appendix No. 2 to SWZ in Part 1 in items 1-4 and two sets for eye marking during toric lens implantation.5For the duration of the contract, the Contractor is obliged, as part of the price of the subject matter of the contract for part 1, to deliver and deposit at the Principal's seat, within 4 weeks from the date of conclusion of the contract, a phacoemulsification and anterior vitrectomy apparatus and a posterior vitrectomy apparatus with the parameters and equipment specified in Appendix No. 2 to the SWZ, and to ensure proper operation of the apparatus throughout the duration of the contract. All operating costs, including inspections, repairs, maintenance and servicing, as well as training of the Ordering Party's personnel in the use of the equipment, must be included in the price of the subject of the contract as regards part 1. Armii Krajowej 101: accessories for cataract operations - part 2; according to the quantity and type list specified in Appendix No. 2 to the Terms of Reference, hereinafter referred to as "SWZ", meeting the quality parameters defined in Appendix No. 2 to SWZ, under the conditions specified in the draft provisions of the agreement constituting Appendix No. 4 to SWZ.2The offered goods have to be new and unused (except for lending apparatus for eye surgery).3. delivery of the subject matter of the contract together with transport and unloading will take place at the expense and risk of the Contractor. The subject of the order is to supply the Bielsko-Biała Voivodship Hospital in Bielsko-Biała, 101 Armii Krajowej Avenue, with: sets for complicated glaucoma surgery - part 3; according to the quantity and type list specified in Appendix No. 2 to the Specification of Order Terms, hereinafter referred to as "SWZ", meeting the quality parameters defined in Appendix No. 2 to SWZ, under the conditions defined in the draft provisions of the agreement constituting Appendix No. 4 to SWZ.2The offered goods have to be new and unused (except for lending apparatus for eye surgery).3. delivery of the subject matter of the contract together with transport and unloading will take place at the expense and risk of the Contractor. The subject of the contract is to supply to the Bielsko-Biała Voivodship Hospital in Bielsko-Biała, 101 Armii Krajowej Avenue: Supplementary accessories for posterior vitrectomy surgery according to the quantity and type list specified in Appendix No. 2 to the Terms of Reference, hereinafter referred to as "SWZ", meeting the quality parameters defined in Appendix No. 2 to SWZ, under the conditions specified in the draft contract provisions constituting Appendix No. 4 to SWZ.2The offered goods have to be new and unused (except for lending apparatus for eye surgery).3. delivery of the subject matter of the contract together with transport and unloading will take place at the expense and risk of the Contractor. The subject of the order is to supply to the Bielsko-Biała Voivodship Hospital at 101 Armii Krajowej Avenue additional accessories for posterior vitrectomy surgery according to the quantity and type list specified in Appendix No. 2 to the Terms of Reference, hereinafter referred to as "SWZ", meeting the quality parameters defined in Appendix No. 2 to SWZ, under the conditions specified in the draft contract provisions constituting Appendix No. 4 to SWZ.2The offered goods have to be new and unused (except for lending apparatus for eye surgery).3.
