REQUEST TO SUBMIT QUOTATION FOR THE SUPPLY AND DELIVERY OF AGRICULTURAL SUPPLIES AND MATERIALS FOR USE AT DA-CNLRRS AT APPROVED BUDGET FOR THE CONTRACT AMOUNTING TO FIFTY FIVE THOUSAND PESOS (PHP55,000.00) PURSUANT TO ALTERNATIVE METHOD OF PROCUREMENT SPECIFICALLY NEGOTIATED PROCUREMENT - SHOPPING UNDER SECTION 52.1 UNDER RA 9184 AND ITS REVISED IRR. The Department of Agriculture Regional Field Office No. 5 (DA RFO 5), San Agustin, Pili, Camarines Sur is inviting interested suppliers to submit complete quotation for the Supply and Delivery of Agricultural Supplies and Materials for use at DA-CNLRRS. The agency intends to apply the amount of Fifty Five Thousand Pesos (Php55,000.00) as the Approved Budget for the Contract. Partial bid is allowed. As per approved Purchase Request and considering the nature of requisition being not more than Two Hundred Thousand Pesos (Php200,000.00) and in order to fast-track the implementation of the program, this procurement is facilitated thru Negotiated Procurement-Shopping under Section 52.1 of RA 9184 and its Revised IRR. As such, CAMARINES VET PRO PLUS, Purok 3, J. Pimentel St., Brgy. V, Daet, Camarines Norte; G FARM SHOP AND GENERAL MERCHANDISE, Diversion Road, Brgy. Camambugan, Daet, Camarines Norte, and LAREM AGRI-VET ENTEPRISES, J. Lukban St., Daet, Camarines Norte and any other qualified/interested supplier are hereby requested to submit the following documents: 1. Mayor’s Permit 2. PhilGEPS Registration 3. BIR Registration The bidders are required to pay the non-refundable amount Php500.00 for bid documents to the Cashier’s Office, DA RFO-5, Pili, Camarines Sur. All submitted documents must be in duplicate, otherwise, bids will be automatically rejected on the ground of non-compliance with the prescribed bid requirements. Sealed quotation must be received by BAC Secretariat, DA RFO 5 not later than 1:30 in the afternoon of 19 November 2018 at DA Library, DA RFO 5, Camarines Sur.
