Description

The subject matter of the contract is the delivery of equipment to the Central Sterilization Department, together with its installation and adaptation of rooms.2 The subject matter of the contract has been described in detail in attachment no. 2a, 2b, 2c to SWZ (description of the subject matter of the contract).3 The Ordering Party allows for the possibility of submitting partial offers. The subject matter of the contract has been divided into 3 parts.4 The Economic Operator is obliged to execute the contract according to the terms and conditions described in the draft provisions of the contract constituting attachment no. 6a and 6b to SWZ5. For parts 1 and 2 - the Contracting Authority requires a site inspection to be held on 28 April 2021 at 10.00 a.m. (attendance is compulsory - meeting in front of the main entrance to the Hospital at ul. 28 Czerwca 1956 r. Absence of the Bidder from the site inspection will result in rejection of his bid in accordance with art. 226 par. 1 item 18 of the PPL Act.6 Term of warranty given by the Bidder - 36 months from the date of signing the acceptance protocol.
The subject matter of the contract is the delivery of equipment to the Central Sterilization Department, together with its installation and adaptation of rooms - Part no. 1 - Steam sterilizer with equipment and adaptation of rooms.2. The subject matter of the contract has been described in detail in appendix no. 2a, 2b, 2c to SWZ (description of the subject matter of the contract).3. The Contractor undertakes to execute the subject matter of the contract with due diligence, in accordance with the documentation, applicable provisions of the Construction Law, technical norms, standards, professional ethics, OSH and fire safety regulations, provisions of the SWZZ, draft contractual provisions and principles of the construction art and with observance of the requirements specified in the documentation.5. 135/147 in Poznań). The Contracting Authority shall not provide any explanations regarding the subject matter of the contract, referring Bidders to requests for clarification of the content of the ToR. The Contracting Authority shall require that Economic Operators participate in a site inspection of the place where the contract is to be performed, in order to become acquainted with the subject matter of the contract and to obtain any information that may be helpful in preparing a tender and performing the contract. Failure by the Economic Operator to attend the site inspection will result in rejection of his bid in accordance with art. 226 item 1 point 18 of the PPL Act.6. Contractor's essential information - equivalent solutions1) Wherever the subject matter of the contract is described by indicating trademarks, patents or origin, the Contracting Authority shall allow the Economic Operator to use solutions equivalent to those described in the documentation on condition that they have at least the same or better technical and functional parameters and do not lower the standards specified in the documentation. The concept of equivalence is also applied when the Contracting Authority described the subject matter of the contract using standards, approvals, technical specifications and reference systems.2) If the Economic Operator proposes equivalent materials and other equivalent elements, they are obliged to prepare and enclose to their offer a list of all the proposed equivalent materials, devices and other elements and prove their equivalence to the materials, devices and other elements described in the documentation constituting the description of the subject matter of the contract, indicating the name, page and item of the technical specification for execution and acceptance of works/schedule of works to which it refers.3) All equivalent materials or other elements proposed by the Contractor must have technical and functional parameters not worse than those specified in the documentation and have the appropriate approvals and certificates.7The offered object of the contract has to be allowed for circulation and use in the territory of the Republic of Poland in accordance with the requirements of the Act of 20 May 2010 on medical devices (i.e. Journal of Laws of 2020, item 186) and European Union directives (in accordance with art. 26 of the Act of 20 May 2010 on medical devices (i.e. Journal of Laws of 2020, item 186)8. The term of the guarantee given by the Contractor - 36 months from the date of signing the acceptance protocol without any comments.9. The subject matter of the contract also includes training of the Ordering Party's personnel in the scope of current operation, maintenance and exploitation of the offered equipment.10. The subject matter of the contract will be delivered at the expense, risk and transport of the Contractor.11. The subject matter of the contract is the delivery of equipment to the Central Sterilization Department, together with its installation and adaptation of the rooms - Part No. 2 - Washer disinfector, together with equipment and adaptation of the rooms.2 The subject matter of the contract has been described in detail in Appendix No. 2a, 2b, 2c to SWZ (description of the subject matter of the contract).3 The Contractor shall execute the order under the terms and conditions described in the draft provisions of the contract, constituting an appendix to SWZ.4The Contractor undertakes to execute the subject matter of the contract with due diligence, in accordance with the documentation, binding provisions of the Building Law, technical norms, standards, professional ethics, OSH and fire safety regulations, provisions of the SWZ (Terms of Reference), draft contractual provisions and principles of the building craft, and with observance of the requirements specified in the documentation.5. 135/147 in Poznań).
