I. Object of the contract1. Name of the subject of the contract: "Delivery of devices, medical equipment and equipment for the extension of the Beskid Oncology Centre - John Paul II Municipal Hospital at 18 Wyzwolenia Street in Bielsko-Biała" - 2 parts of the procedure.2 The subject of the contract will be implemented as part of the project: "Modern base of the Beskid Oncology Centre - John Paul II City Hospital in Bielsko-Biała" co-financed from the funds of the "Regional Operational Programme of the Silesian Voivodeship for the years 2014-2020 priority axis: X. Revitalisation and social and health infrastructure for action: 10.1 Health care infrastructure".3 The General Contractor for the extension of the Beskid Oncology Centre - John Paul II Municipal Hospital in Bielsko-Biała at 18 Wyzwolenia Street (hereinafter: user) is a consortium of companies: Alstal Grupa Budowlana Sp. z o. o. Sp. Komandytowa and Alstal Development Sp. z o.o. (hereinafter referred to as: General Contractor).
a) The subject matter of the contract is the delivery, assembly and commissioning of devices and technological equipment of the sterilization room for the needs of the Beskid Oncology Centre - John Paul II Municipal Hospital in Bielsko-Biała. b) A detailed description of the subject matter of the contract is included in Appendix 3.c) Name(s) and code(s) of the Common Procurement Vocabulary - CPV:- 33191000-5 Sterilization, disinfection and hygiene equipment - 33192000-2 Medical furniture) Pursuant to art. 45 item 1 of the PPL Act, the Awarding Entity shall demand from the Economic Operators to pay a deposit in the amount of PLN 1,000: PLN 70,000.00 1) Part 2 - Macerators of disposable vessels (ducks, swimming pools)a) The subject matter of the contract is the delivery, assembly and start-up of disposable vessel macerators for the needs of the Beskid Oncology Centre - Municipal Hospital. b) A detailed description of the subject matter of the contract is included in Annex 3.2 to the ToR, which constitutes an integral part thereof and the Economic Operator is obliged to get acquainted with it before submitting a tender.c) Name(s) and codes of the Common Procurement Vocabulary - CPV:- 42996100-5 Shredders - 42996110-8 Wastewater treatment macerators
