MAINTENANCE CLEANING OF SOLAR PANELS - 44.00 units Required Equipment: 1 – unit Boomtruck (3 Tons) (owned/leased) 1 – unit Electric Drill (owned) Approved Budget for the Contract (ABC) : P 3,484,191.00 Source of Fund : PPA R/M Funds Contract Duration : 60 Calendar Days Required PCAB Registration : Small“B”–Electrical Works 2 The schedule of procurement activities are as follows: ACTIVITIES DATE Posting of Request for Quotation November 21, 2018 Issuance and Availability of RFQ and other Procurement Documents November 21, 2018 to Nov. 28, 2018 Preliminary Conference/Clarification November 26, 2018, 10:00 AM Deadline for submission of RFQ and Technical Requirements November 28, 2018, 9:00 AM Opening of Request for Quotation November 28, 2018, 10:00 AM 3 Supplier is hereby invited to submit its sealed quotation/proposals to BAC Secretariat, PPA PMO-Surigao, Admin Building, Port Area, Surigao City on or before set deadline above. 4 Interested supplier/contractor must present Original Copy of Special Power of Attorney (SPA)/ Secretary’s Certificate from the owner/corporation if done by a representative. A complete set of documents may be purchased by interested contractor/s from the Secretariat upon payment of a non-refundable fee of Five Thousand Pesos (P 5,000.00) inclusive of VAT. 5 Copies of the following eligibility, technical and financial documents are also required to be submitted together with the quotation/proposal: A. ELIGIBILITY DOCUMENTS Class “A” Documents a) DTI business name registration or SEC registration documents b) Valid and current Mayor’s permit/Municipal license c) Valid Tax Clearance d) Certificate of PhilGeps registration e) Valid PCAB license & registration for the type and cost of the contract posted f) Audited financial statements, stamped, received by the BIR of its duly accredited and authorized institutions, for the immediate preceding calendar year, showing among others the total and current assets and liabilities B. TECHNICAL DOCUMENTS a) Duly Signed Omnibus Statement b) Proof of ownership/ lease agreement for equipment C. FINANCIAL DOCUMENTS a) Duly signed and Accomplished Proposal Form 6 The PPA, PMO-Surigao reserves the right to accept or reject any bid, to annul the bidding process, and to reject all bids at any time prior to contract award, without thereby incurring any liability to the affected bidder or bidders.
