Description

The subject matter of the contract is the performance of repair, inspection and maintenance services of ventilation and air-conditioning equipment built in the facilities of University Hospital No. 1 in Bydgoszcz, specifically specified in Annexes 1a and 2 to the Terms of Reference, hereinafter referred to as the ToR. The subject matter of the contract includes 8 indivisible parts for which the Contracting Authority allows partial tenders, with the proviso that a tender in each of them should be complete and should meet the detailed requirements specified in the price forms constituting Annex 2 to the ToR, as well as the requirements contained in Annexes 1a and point 3 to the ToR.
The subject matter of the contract is the inspection, maintenance and repair of ventilation equipment in buildings C, D, E, F, I, H, referred to in Annexes 1a and 2 to these Terms of Reference, hereinafter referred to as the ToR. Detailed requirements are specified in the price forms, constituting Annex 2 to the ToR, in Annex 1a and in point A of the ToR. 3 to ToR.6.2 Contracts may be tendered for by Economic Operators who fulfil the conditions set out in Article 22, paragraph 1.b of the PPL Act, concerning:c) technical or professional capacity; the Economic Operator shall meet the minimum conditions concerning professional qualifications, experience, technical capacity of the economic operator or persons delegated by the economic operator to perform the contract, allowing for performance of the contract at an appropriate quality level, if it proves that:For part no. 1:a) has duly performed or will perform in the last three years before the end of the period of validity of the contract, and if the period of activity is shorter, two contracts for maintenance and/or commissioning and/or installation and/or servicing of air handling units, for a total value not lower than PLN 240,000.00; b) has or will have at its disposal, throughout the entire term of the contract, at least 2 persons of technical staff who will be referred by the Economic Operator to perform the contract, and in particular will be responsible for providing services, and have the following professional qualifications, i.e.:- valid certificates of qualification Group E1 pt. 2 "electrical equipment, installations and networks not higher than 1kV", in accordance with the provisions of the Regulation of the Minister of Economy, Labour and Social Policy of 28 April 2003 on detailed rules of confirming qualifications by persons involved in the operation of equipment, installations and networks (Journal of Laws of 2003 No. 89 item 828 7.2 For the purpose of confirming that the conditions for participation in the procedure referred to in item 6.2 of ToR, the Awarding Entity shall demand from the Economic Operator whose tender is going to be the highest evaluated, the following documents: For part no. 1:a) the list of services rendered (a specimen is enclosed as Annex 4 to ToR) within the last three years before the deadline for submission of tenders, and if the period of conducting business activity is shorter - within this period referred to in item 6.2 of ToR, the Awarding Entity shall demand from the Economic Operator whose tender is going to be the highest evaluated, the following documents 6.2 c) To ToR for part No. 1, including their value, subject matter, dates of performance and entities for whom services were performed, and enclosing evidence specifying whether these services were performed or are performed properly, with the evidence in question being references or other documents issued by the entity for whom services were performed, and if for a justified reason of objective nature the contractor is not able to obtain these documents - a statement by the contractor; in the case of periodic or continuous services, references or other documents proving their proper performance should be issued not earlier than 3 months before the deadline for submission of tenders; b) the list of persons (specimen constitutes Annex No. 4a to the ToR) referred to in point A of the ToR. 6.2 c) ToR for part no. 1, addressed by the Economic Operator for the performance of a public contract, in particular those responsible for providing services, with information on their professional qualifications, experience and education necessary to perform the public contract, as well as the scope of their activities and information on the basis for their disposal. The following may be applied for by Economic Operators who meet the requirements specified in art. 22, par. 1b of the PPL, concerning: a) competence or rights to run a given department. Detailed requirements are specified in the price forms, state of Annex No. 2 to ToR, Annex No. 1a and point 3 of the ToR. 3 OF THE TOR. 6.2 The Contracting Authority may be applied for the contract by Economic Operators who meet the requirements defined in art. 22 par. 1b of the PPL, concerning: a) competence or rights to determine professional activity, i.e.: the Economic Operator, throughout the whole period of the contract obligation, must hold a valid certificate for entrepreneurs conducting business activity and perform activities for third parties. installation, maintenance or servicing, repair or liquidation of: stationary refrigeration equipment, air-conditioning system, valves. fluorinated greenhouse gases, as referred to in Article 2(2) of the Work Programme. In order to be able to participate in the progress referred to in point E1.2. 6.2 of the ToR, the Contracting Authority shall request from the Contractor who will be the highest rated bidder, before the following doc. a) concerning the competence or rights to the profession, if it results from separate regulations, i.e.: Certificate for entrepreneurs issued by the Office of Technical Inspection (UDT) pursuant to art. 30 par. 7 of the Act of 15.05.2015 on depleted ozone layer substratum. and certain fluorinated greenhouse gases, or equivalent issued by another EU entity, the so-called Cert. operate. The Contractor shall be responsible in particular for the provision of services and shall have the following professional qualifications: valid certificates of qualification. The Contractor shall be an authorized representative of the manufacturer of Control and Measurement Instruments and Automation devices together with Schneider controllers as of the date of submission of offers and throughout the entire contract period or have the manufacturer's authorization to carry out inspection, maintenance and repair of Control and Measurement Instruments and Automation devices together with Schneider controllers.
