Description

(1) The subject of the contract is the supply of oncological endoprostheses described in the following parts: - Part 1 - Oncological, unresectable, modular upper limb endoprostheses, - Part 2 - Oncological, unresectable, modular lower limb endoprostheses, - Part 3 - Knee-joint atrodesis, - Part 4 - Non-cemented, cemented modular revision endoprosthesis.2.The medical products offered must meet the essential requirements set out for such products in the relevant regulations, bear the CE mark and be admitted to marketing and use in the territory of the Republic of Poland in accordance with the Medical Devices Act of 20 May 2010.3 A detailed list, requirements, quantities and description of the subject matter of the contract are contained in Annex 2 to the SWZ.
The subject of the contract is the supply of oncological, post-resection, modular endoprostheses of the upper limb. Detailed list, requirements, quantities and description of the subject matter of the contract are contained in appendix no. 2 to SWZ.2In accordance with Article 13 (1) and (2) of the Regulation (EU) 2016/679 of the European Parliament and of the Council of 27 April 2016 on the protection of individuals with regard to the processing of personal data and on the free movement of such data, and repealing Directive 95/46/EC (General Data Protection Regulation) (Dz.OJ EU L 119, 4.5.2016, p. 1), hereinafter referred to as "RODO", I inform you that:2.1. the controller of your personal data is the Independent Public Health Care Facility of the Ministry of Interior and Administration with the Warmia and Mazury Oncology Centre based in Olsztyn at al. Wojska Polskiego 37;2.2. personal data protection inspector in SP ZOZ MSWiA with W-MCO in Olsztyn is Mr. Jarosław Łęgowski, contact: e-mail address, abi@poliklinika.net;2.3. your personal data will be processed on the basis of art. 6. c RODO for the purpose related to the proceedings for the award of a public contract marked ZPZ-11/03/21 entitled: Delivery of oncological endoprostheses;2.4. the recipients of your personal data will be persons or entities to whom the documentation of the proceedings will be made available on the basis of Article 18 and Article 74 of the Act of 11 September 2019. Public Procurement Law (Journal of Laws of 2019), hereinafter referred to as the "PPL Act";2.5. your personal data will be kept, pursuant to Article 78(1) of the PPL Act, for a period of 4 years from the date of completion of the procurement proceedings;2.6. the obligation for you to provide personal data directly concerning you is a statutory requirement under the PPL Act related to participation in the public procurement proceedings; consequences of failure to provide certain data arise under the PPL Act;2.2.7. with regard to your personal data no decisions will be taken in an automated manner pursuant to Article 22 RODO;2.8. you have the following: - pursuant to Article 15 RODO the right to access personal data concerning you,- pursuant to Article 16 RODO the right to rectify your personal data *,- pursuant to Article 18 RODO the right to demand from the controller the restriction of processing of personal data right to lodge a complaint to the President of the Office for Personal Data Protection, if you consider that the processing of personal data concerning you violates the provisions of RODO;2.9. 2.9. you are not entitled to: - in relation to Article 17(3)(b), (d) or (e) of RODO the right to erasure of personal data,- the right to the portability of personal data referred to in Article 20 of RODO,- pursuant to Article 21 of RODO the right to object to the processing of personal data, because the legal basis for the processing of your personal data is Article 6(1)(c) of RODO.* Explanation: exercising the right to rectification shall not change the result of the public procurement procedure or the provisions of the contract to an extent contrary to the PPL Act and shall not affect the integrity of the protocol and its appendices ** Explanation: the right to restrict processing shall not apply to storage, to ensure the exercise of legal remedies or to protect the rights of another natural or legal person, or on account of important grounds of public interest of the European Union or of a Member State.
