Description

1. the subject of the contract is: Collection, transport and disposal of medical waste with codes 18 01 02, 18 01 03, 18 01 04, 18 01 08, 18 01 09.
1) Classification symbols of the subject of the contract are given in accordance with the ordinance of the Minister of Climate of 2 January 2020 on the catalogue of waste (Journal of Laws 2020 item 10).2) The Ordering Party requires the Contractor to ensure safety of service performance at all stages of medical waste handling (collection, transport and disposal) in accordance with the following legal regulations applicable in the Republic of Poland:- the Act of 27 April 2001. Environmental Protection Law (consolidated text Dz.U. of 2019, item 1396),- Act of 14 December 2012 on waste (Dz.U. 2020, item 797 as amended),- Act of 19 August 2011 on transport of dangerous goods (consolidated text Dz.U. of 2020 item 154),- the regulation of the Minister of Health of 21 October 2016 on the requirements and methods of neutralisation of medical and veterinary waste, Journal of Laws of 2016, item 1819,- the regulation of the Minister of Health of 5 October 2017 on the detailed method of handling medical waste (Journal of Laws 2017, item 1975),- the regulation of the Minister of Health of 24 July 2015 on the types of medical and veterinary waste whose recovery is acceptable (Journal of Laws of 2015, item 1819), item 1116),- the regulation of the Minister of the Environment of 13 January 2014 on the document confirming neutralization of infectious medical or veterinary waste (Journal of Laws of 2014, item 107 as amended),- the regulation of the Minister of the Environment of 7 October 2016 on detailed requirements for waste transport (Journal of Laws of 2016, item 1742).3) The contract covers the following scope:a) collection, loading, transport and utilization of medical waste with codes 180102, 180103, 180104, 180108, 180109; b) average monthly quantity of waste with codes 180102, 180103 will be about 40 00 kg, for the whole contract period - 48 000 kg; c) average monthly quantity of waste with codes 180104 will be about 3 000 kg, for the whole contract period - 36 000 kg;d) collection of waste codes 180102, 180103, 180104 - every 48 hours (Mondays, Wednesdays, Fridays); however, the storage time of waste code 18 01 02 may not be longer than 72 hours, which may result in the Contractor being obliged to collect waste also on a public holiday; e) collection of waste codes 180108, 180109 - not more often than once a month from the hospital pharmacy, at 7 a.m.00-15.00, after prior notification by employees of the hospital pharmacy; f) waste will be collected in a special warehouse, in plastic bags - maximum weight of 20 kg; g) each collection must be confirmed by a waste transfer card, on which the waste code and its weight must be given.4) The obligations of the Contractor include preparing the consignment (medical waste) and performing its transport and related loading and unloading in accordance with the requirements applicable to the transport of hazardous materials, and transferring the waste to a disposal plant for destruction.5) From the moment of waste collection, the Contractor takes full responsibility for damage caused by its loss or improper protection and illegal method of disposal.6) The Contractor is obliged to transport waste in accordance with the regulations on the transport of dangerous goods, specialized, monitored (e.g. GPS) means of transport, with a capacity ensuring systematic collection of waste from the Customer, operated by qualified personnel.
