Description

1) The subject matter of the contract is the supply of medical equipment for the intensive care room of the Division of Internal Medicine and Gastroenterology within the framework of the project "Highest quality and safety of services to prevent cardiotoxicity in chemotherapy treatment" divided into 7 parts.2) A detailed description of the subject matter of the contract is provided in attachment no. 2 to the SWZ - a form for the required technical parameters.3) The Ordering Party informs that wherever the subject matter of the contract is described in the SWZ by indicating trademarks, patents orThe Contracting Authority hereby informs that wherever in the SWZ (Terms of Reference) it describes the subject matter of the contract by indicating trademarks, patents or origin, sources or a specific process that characterizes the products or services provided by a particular contractor, which could lead to preference or elimination of certain contractors or products, the Contracting Authority shall allow solutions equivalent to those described provided that they are of no worse properties and quality.
The subject matter of the contract is the supply of medical equipment (ECG - 12 channels - 1 piece) for the intensive care room of the Department of Internal Medicine and Gastroenterology within the framework of the project "Highest quality and safety of service to prevent cardiotoxicity in chemotherapy treatment".2.. In accordance with Article 13(1) and (2) of the Regulation of the European Parliament and of the Council(EU) 2016/679 of 27 April 2016 on the protection of natural persons with regard to the processing of personal data and on the free movement of such data, and repealing Directive 95/46/EC(General Data Protection Regulation) (Dz. Urz. EU L 119, 04.05.2016, p. 1), hereinafter referred to as "RODO", I inform that:3.1. the controller of your personal data is the Independent Public Health Care Facility of the Ministry of Interior and Administration with the Warmia and Mazury Oncology Centre with its registered office in Olsztyn at Wojska Polskiego Avenue 37;3.2. The personal data protection inspector at the SP ZOZ MSWiA with W-MCO in Olsztyn is Mr Jarosław Łęgowski, contact: e-mail address, abi@poliklinika.net; 3.3. Your personal data will be processed on the basis of Article 6.1.c RODO for the purpose of the public procurement procedure ZPZ-15/03/21 titled: Delivery of medical equipment for the intensive care room.: Delivery of medical equipment for the intensive care room of the Department of Internal Medicine and Gastroenterology as part of the project "Highest quality and safety of service provision to prevent cardiotoxicity in chemotherapy treatment".3.4. the recipients of your personal data will be persons or entities to whom the documentation of the proceedings will be made available on the basis of Article 18 and Article 74 of the Act of 11 September 2019. - Public Procurement Law (Journal of Laws of 2019), hereinafter referred to as "PPL";3.5.
