Description

1 The subject of the order is the lease of a biochemical-immunochemical platform along with the supply of reagents, calibrators, controls, flushing fluids and consumable parts. (2) A detailed description of the subject matter of the order is contained in Annex No. 2 - assortment and price form and Annex No. 3 - list of technical parameters. (3) The offered medical equipment should meet the requirements specified in the Act of April 7, 2022 on medical devices (Journal of Laws 2022, item 974, as amended). 4 The offered platform must be manufactured no earlier than 2022, complete, free of defects in design, materials and workmanship.
(1) The subject of the contract is the lease of a biochemical-immunochemical platform together with the supply of reagents, calibrators, controls, flushing fluids and consumable parts. (2) A detailed description of the subject matter of the order is contained in Annex No. 2 - assortment and price form and Annex No. 3 - list of technical parameters. (3) The offered medical equipment should meet the requirements specified in the Act of April 7, 2022 on medical devices (Journal of Laws 2022, item 974, as amended). 4. the offered platform must be manufactured no earlier than 2022, complete, free of defects in design, materials and workmanship. 5. the offered subject of the contract must have the CE mark, be in accordance with the requirements of the Employer described in the terms of reference and with the laws in force in Poland, have the required declarations of conformity, attestations, certificates, certificates of registration, certificates of approval for marketing and use in Polish health care facilities based on the laws in force in this regard. 6. the Contractor shall bear the cost of delivery and insurance, as well as the responsibility for the delivery of the subject of the order to Novodvork Medical Center. (7) Deliveries of reagents, calibrators, controls, flushing fluids and consumable parts shall be made according to the needs of the Ordering Party in quantities and frequency agreed upon by the order, sent by e-mail. (8) The Contractor undertakes to deliver reagents, calibrators, controls, flushing fluids and consumable parts within a maximum of 5 days from the date of order notification. (9) The subject matter of the order shall be delivered to the laboratory of the Nowodworski Medical Center in Nowy Dwor Mazowiecki, 2 Miodowa Street, on working days from 8:00 a.m. to 8:00 p.m. (10) The quantities given in Appendix No. 2 to the SWZ are indicative only and may change during the execution of the contract due to the needs of the Ordering Party. The Ordering Party undertakes to realize not less than 50% of the value of the contract. (11) The Contractor undertakes to deliver and install the platform within 20 to 30 calendar days from the date of conclusion of the contract to the laboratory of the Nowy Dwor Mazowiecki Medical Center in Nowy Dwor Mazowiecki, 2 Miodowa Street, on working days. 12 Delivery of the leased platform includes: - installation and assembly of the platform at the Purchaser's site - commissioning - providing the Purchaser with an instruction manual in Polish - staff training. 13.After installation and commissioning, the device must be fully operational without any additional costs for this by the Ordering Party. The Contractor shall, at its own expense, train personnel on the use of the supplied equipment.
