Description

Construction works "Rebuilding and renovation of obstetrics and neonatal intensive care subunits together with the necessary technical infrastructure for the Perinatology Centre at the St. Hedwig Królow Hospital No. 2 in Rzeszow" carried out under the project entitled "Improving the quality and availability of medical services at the St. Hedwig Królow Hospital No. 2 in Rzeszow". The detailed scope of works being the subject of the contract is specified by: project documentation of the task, technical specifications for the execution and acceptance of works and the quantities of works, which are only auxiliary material - Annex no. 1 to the Specification of Essential Conditions of the Contract.
Performance of works "Reconstruction and repair of midwifery subdepartments
and intensive care of the newborn with the necessary technical infrastructure for the needs of the Perinatology Centre in the St. Hedwig Królow Hospital No. 2 in Rzeszow" carried out under the project entitled "Improving the quality and availability of medical services in the St. Hedwig Królow Hospital No. 2 in Rzeszow". The detailed scope of works being the subject of the contract is specified by: project documentation of the task, technical specifications for the execution and acceptance of works and the quantities of works,
which are only auxiliary material - Annex 1 to the Terms of Reference. The subject of the investment is a construction project involving reconstruction and general overhaul of the neonatal intensive care unit of the Military Hospital No. 2 in Rzeszów, located in the eastern wing of the first floor of building 1A, as well as reconstruction of selected electrical chessboards and main electrical switchboard in building "A" together with designed new DN 25 hydrants (larger boxes), replacement of risers and levels of sanitary installations. The investment is initially divided into 3 stages of implementation: of which the third stage was completed. This documentation includes updates of stages I and II from 2013, which will now be implemented in one stage. The investment task as a whole is in progress. The reconstruction and repair area is the subject of the order: The investment task as a whole is in progress.The reconstruction and renovation area which is the subject of the order: 895.67 m2.The place of realization: low, high ground and first floor rooms of the hospital building 1A. Building 1A is a part of the combined complex of hospital buildings located on plot no. 56/34, 219 Rzeszów - Wilkowyja Południe. Access to the building from Armii Krajowej Street on the western side of the hospital complex. The Contracting Authority provides for the possibility of awarding similar contracts referred to in Article 67 section 1 item 6 of the Act consisting in the repetition of similar works in relation to the works being the subject of the contract, constituting not more than 30% of the gross value of the basic contract. The contract shall be awarded provided that the Contracting Authority has the necessary funds and, in particular, the following conditions are met:1. the Contractor executed the contract on time and with the utmost care;2. the Contractor shall ensure no worse standard of performance of a new contract than the basic one;3. the Contractor shall accept the essential conditions of the existing contract, as well as the parties, as a result of negotiations, shall agree on the remuneration and deadline for performance of such a contract;4. the nature of the works shall not differ from the basic contract;5. the scope of works of a similar nature shall be properly justified.
