Description
The subject matter of the contract is provision of occupational medicine services for Poczta Polska S.A. employees through occupational medicine units located in Lublin, Chelm, Zamosc and Biała Podlaska (4 parts).
The subject matter of the order is the provision of occupational medicine services for employees of Poczta Polska S.A. by occupational medicine facilities located in Lublin.2. detailed OPZ is included in Appendix no. 2.1 to the SWZ and in Appendix no. 6 to the SWZ.3. detailed rules for the execution of the subject matter of the order, have been specified in the draft agreement constituting Appendix no. 3 to the SWZ.4.The basic order will include the following number of examinations:City - LublinThe type of examinations - Preventive examinationsThe estimated number to be examined the basic order - 54805.The number of Employees/candidates for work directed to be examined in particular types of examinations, indicated in Attachment No. 1 to the Agreement, is subject to change. The Employer, by written annex to the Agreement, reserves the right to reduce or increase the number of Employees/job candidates referred for examination, as specified in Attachment No. 1 to the Agreement, depending on the actual needs of the Employer's organizational units, which shall consist of the right to demand an increase in the maximum number of Employees/job candidates referred for examination indicated in Attachment No. 1 to the Agreement. No. 1 to the Agreement and to make a corresponding reduction in the maximum number of Employees/candidates for work referred for examination indicated in Annex No. 1 to the Agreement, while maintaining the maximum remuneration specified in paragraph 4 of the Agreement and maintaining the unit prices for a given examination, with the proviso that the increase in the maximum number of Employees/candidates for work referred for examination indicated in Annex No. 1 to the Agreement shall not exceed 100% of this number in total. (6) The amount of remuneration will be changed in the event of any change in the provisions indicated in Article 436.4.b of the PPL, when such changes will affect the cost of performance of the service by the Contractor, i.e.:1) in the event of a change in the rate of tax on goods and services;2) in the event of a change in the amount of the minimum wage for labor or the amount of the minimum hourly rate, established on the basis of the provisions of the Act of 10.10.2002 on the minimum wage for labor;3) the rules for being subject to social insurance or health insurance, or the rate of premiums for social or health insurance;4) the rules for collecting and the amount of payments to employee capital plans, as referred to in the Act of October 4 on employee capital plans.7The contracting authority envisages the possibility of awarding to the existing service provider, under a sole-source contract, pursuant to Article 388(2)(c) of the Public Procurement Law, a contract consisting in the repetition of similar services, consistent with the subject matter of the contract, particularly in the event that it is necessary to continue the contract in order to secure the needs of the contractor, in particular when the amount specified in the contract is exhausted or the period for which the contract was concluded expires.1) Contracts will be awarded under the terms and conditions described in the OPD for a given part with modifications resulting from the needs of the Contracting Officer, but not exceeding the requirements described in the OPD and the draft contract for a given part.2) The total period of execution of contracts awarded under art. 388 item 2 letter c of the Public Procurement Law will not exceed a period of 12 months for a given part of the contract.3) The award of a contract under art. 388 item 2 letter c of the Public Procurement Law will be conditional on:- due performance by the Contracting Authority of the basic contract,- the availability of funds by the Contracting Authority for its implementation. 4) The Contracting Authority will award the contract under art. 388 item 2 letter c of the Public Procurement Law on the terms and conditions resulting from the draft agreement attached to the Terms of Reference, while the subject of negotiations with the Contracting Authority will be, in particular, the price for the execution of the subject of the contract. 5) The contractual penalties will be provided for in the same cases as in the contract concluded in the proceedings for the basic contract,6) The obligations of the W-ca and the Z-ca will be regulated on terms analogous to the contract concluded in the post for the basic contract.
