This request for quote was extended to 6 Jun 2024, 2PM CST for the following reason.
1. The SOW now dated 31 May 2024 wording "Domestic" has been changed to "Per FAR 252.225-70001 Buy American and Balance of Payments Program". Also highlighted in yellow.
2. As part of your quote's technical part response and in addition to already requested information, you must acknowledge/concur or certify that chain/securements/shackles used will abide per FAR 252.225-70001 Buy American and Balance of Payments Program. Failure to provide acknowledgement may render you quote technically unacceptable.
The request for quote due date has been extended to 6 June 2024, 2PM CST
Questions and Answers continuation:
Question 15: We might be missing it, but where do we put the bid price on the document. I see Form 1449 where it is a RFQ. Do you want us to put the total price on the Schedule of Supplies form?
Answer to Question 15: On Solicitation Standard Form 1449 page 3 of 43, insert the unit Price and Amount for each item. Please multiply the unit price by the quantity to get the Amount for each item.

ADDENDUM to FAR 52.212-1 INSTRUCTIONS TO OFFERORS--COMMERCIAL ITEMS
Provisions that are incorporated by reference by FAR 52.252-1, have the same force and effect as if they were given in full text. Upon request, the Contracting Officer will make their full text available.
The following provisions are incorporated into 52.212-1 as an addendum to this solicitation:
Submission of Quotations: Quotes shall be submitted by the due date and time provided on the SF1449 block 8, CST. Offers shall be emailed to SSgt Ian Hinojosa ian.hinojosa_guillen@us.af.mil and Nelson Kraemer nelson.kraemer@us.af.mil.
One (1) electronic copy shall be emailed as follows:
Tab A – Quotation Form (Standard Form 1449) – original signature and date; Contract Administration Data; UEI Number; Acknowledgement of Solicitation Amendments
Tab B – PRICE PROPOSAL: Pricing shall be submitted using the Schedule of Supplies/Services within the Quotation Form (SF 1449).
Tab C - Completed copy of representations and certifications at FAR 52.212-3.
Tab D - CHRONOLOGICAL TECHNICAL APPROACH: The chronological technical approach shall address each factor in 52.212- 2 and clearly identify which factor is being addressed. Utilization of labor, experience, position descriptions, time, material and equipment must be included in the chronological technical approach plan describing your step-by-step procedure/approach. Do not only copy and paste from the statement of work and solicitation. Provide your own company's ideas and solution to the issue. Chronological technical approach shall be no longer than 3 pages, using 12 size font and Times New Roman font style.
Tab E – PAST PERFORMANCE PROPOSAL: Quotations shall provide their past performance. The past performance shall include recent and relevant contracts for the same or similar items. Each identified contract shall include the following information: contract numbers, points of contact of Government personnel who accepted/received the products/services, phone numbers and email addresses for those Government personnel, a description of the requirement and an explanation of how it relates to the current requirement. Recent contracts are those contracts with ordering periods or deliveries occurring, at least in part, within the last three (3) years. Relevance, for the purposes of this procurement, is defined as substantially similar to the current requirement in terms of effort, complexity and contract type. Offerors shall provide no more than three (3) examples of past performance. The past performance information shall be no longer than 5 pages, using 12 size font and Times New Roman font style.
QUESTIONS AND ANSWERS (Q&A) PERIOD The due date for all questions associated with this solicitation is no later than 24 May 2024.
SITE VISIT: Has been established for 22 May 2024 at 1000 CST. Any vendors interested in attending the site visit will need to send the below information to SSgt Ian Hinojosa ian.hinojosa_guillen@us.af.mil and Nelson Kraemer nelson.kraemer@us.af.mil. no later than 16 May 2024 0900 CST. Your participation in the site visit is strictly voluntary on your part. The United States Air Force assumes “NO” liability or expense on behalf of your attendance or participation of the Site Visit.
