Subcontractor6.4.1. The Contracting Authority demands that the Economic Operator indicate a part of the contract and provide subcontractor companies, if known (it should be indicated in the uniform European contract document constituting Annex 3 to the ToR).6.5.1 List of declarations or documents submitted by the Economic Operator in the procedure at the Contracting Authority's request in order to confirm the circumstances referred to in art. 25 par. 1 item 3 of the PPLa Act) information from the National Criminal Register within the scope defined in art. 24 par. 2 of the PPLa Act. b) declaration of the economic operator on the lack of a final court judgment or a final administrative decision on arrears in the payment of taxes, charges or social or health insurance contributions, or - in the event of such a judgment or decision - documents confirming the payment of these charges together with interest or fines, if any, or the conclusion of a binding agreement on the payment of these charges;(d) an extract from the appropriate register or from the central register and information on economic activity, where separate provisions require entry in the register or register to confirm that there are no grounds for exclusion pursuant to Article 2. 24 section 5 item 1 of the PPL Act; e) If the economic operator has its seat or place of residence outside the territory of the Republic of Poland, instead of the documents referred to in:1)(a) - it shall submit information from a relevant register or, in the absence of such a register, another equivalent document issued by a competent judicial or administrative authority of the country in which the economic operator has its seat or place of residence or the person to whom the information or document relates, to the extent specified in art. 24 (1) (13), (14) and (21) of the PPL Act;2) (d) - submit a document or documents issued in the country where the economic operator has its registered office or place of residence, confirming that no liquidation has been opened or declared bankrupt.3) The documents referred to in points 1) and 2) should be issued not earlier than 6 months before the deadline for submission of tenders.4) If the documents referred to in point 1 are not issued in the country in which the economic operator has its registered office or place of residence or the person concerned by the document, they shall be replaced by a document containing, as appropriate, a statement by the economic operator, indicating the person or persons authorised to represent him, or a statement by the person concerned by the document, made before a notary public or before a judicial, administrative or professional or economic self-government body competent in respect of the economic operator's registered office or place of residence.5) The Economic Operator having the seat within the territory of the Republic of Poland, with reference to a person residing outside the territory of the Republic of Poland, to whom the document indicated in point a applies, shall submit the document referred to in point 1 within the scope defined in art. 24 item 1 points 14 and 21 of the PPL Act. If in the country where the person to whom the document was to apply is domiciled the document is not issued, it shall be replaced by a document containing a declaration by that person before a notary public or before a judicial, administrative or professional or economic self-government body competent in respect of the place of residence of that person.Continued in Section II.2.4. of Part No. 2. 1.
