Description

Purchase and delivery with installation of a computer tomograph within the framework of the task entitled "Małopolska Anti-Crisis Shield - Medical Package 2" together with a special purpose grant from the Municipality of Krakow in accordance with the description of the subject of the order and the requirements included in Appendix 1 to the ToR and Appendix 1a and 1b.
Purchase and delivery of 1 piece of a CT scanner together with assembly, commissioning and acceptance tests. and design of the x-ray shields together with training of the employees; according to the list of conditions and parameters required as appendix 1 to the specification and appendices 1a and 1b. The contractor will perform:1. Disassembly and disposal of the existing equipment2. painting the workshop3. Replacing the carpet4. possible execution of additional channels (in case of a small number of channels it is allowed to install trays on the wall)5. Design of radiological shields6. If it is necessary to use additional shields (walls, windows and doors and windows between the studio and the control room), the works in the Remote diagnostics of the system via broadband or ISDN connection should be included in the offer. Performing acceptance tests and specialist tests (including tests on monitors) after installation of the device for the offered X-ray set in accordance with the currently binding Regulation of the Minister of Health (separate protocols for acceptance and specialist tests). Performing acceptance tests after significant warranty repairs. Integration of the hardware and software delivered in the course of the proceedings with the hospital's RIS system and the hospital's PACSGGuarantee min. 36 months (counted from the date of signing the protocol of acceptance of the subject of the order) Periodic inspections every 12 months, unless the manufacturer recommends more often with replacement of wear parts, including one at the end of the warranty period. also with replacement of wear parts and materials recommended during the inspection. After each inspection, issue a certificate of fitness and make an appropriate entry in the technical passportEvery repair of the warranty.extends the warranty period for the total duration of the repairVerification of the complaint by phone, e - mail or fax. within 24 hours of its sending by the Purchaser.on working days counting from Monday - Friday from statutory holidays onlyThe service response time (accepted application - repair undertaken) - up to 48 h max. The "undertaken repair" includes the presence of an authorized contractor's employee at the damaged unitSwap the damaged component for a brand new one within 6 days of its failure by the service engineer in the presence of an order representative. The maximum repair time cannot exceed 7 days from the acceptance of the notification. 7-year access to spare parts, operating materials and accessories from the date of saleTraining of medical personnel in the scope of operating the equipment on two dates (about 5 people each); Service documentation for the purposes of the Order. - Upon delivery of the device and its independent equipment, the contractor will provide a Passport containing at least the following data: name, type (model), manufacturer, Year of manufacture, serial number (factory), other relevant information (e.g. component parts, relevant equipment), codes from the currently valid NFZ dictionary (if any), Operating Instructions in Polish in the form of electro and printed (upon delivery) Year of manufacture: 2021Medical products must meet the requirements of the Act of 20 May 2010 on Medical Devices (Journal of Laws 2017, item 211 as amended), i.e. they have entries and certificates issued by authorized bodies: the manufacturer's CE declaration of conformity (applies to all classes of medical device), the certificate of the notified body (concerning the class of medical device): The order informs about the possibility of conducting the local inspection in the room where the tomograph is to be installed by the contractors. The contract informs that it does not provide for a meeting of the contractors in order to clarify doubts concerning the contents of the ToR, and during the on-site visit it will not provide answers to questions concerning the contents of the ToR. The Contracting Authority hereby informs that participation in the above mentioned visit is not obligatory within the meaning of art. 9a of the Act, i.e. bids may be submitted without the contractor performing the on-site visit.
