"2 A detailed description of the subject matter of the contract is provided in Appendix No. 1 to the SWZ.3ATTENTION!The Ordering Party informs that it is necessary for the contractors to participate in a site inspection at the place of installation of the CT scanner. If the Contractor fails to participate in the site inspection, their offer shall be rejected pursuant to art. 226 sec. 1 item 18 of the Public Procurement Law. In order to participate in the site inspection, contractors shall be obliged to notify the person authorized to contact the contractors of the names of the persons who will participate in the site inspection 2 working days before the scheduled date at the latest to the e-mail address molenda.m@powiat.kielce.pl The on-site visit will be conducted from November 15 to 17 at 12:00 p.mPersons arriving are required to carry an identity document. The purpose of the site inspection is to familiarise oneself with the technical condition of the rooms, the electrical connection and to determine the scope of the necessary adaptation works.4 The supplied equipment shall comply with applicable standards and shall have the required documents confirming that the subject matter of the contract has been authorised for marketing and use in Poland in accordance with applicable EU Directives and in accordance with the requirements of the Medical Devices Act of 20 May 2010 (i.e. Journal of Laws of 2020, item 186, as amended) and CE Certificates.5As part of the contract, the Contractor shall be obliged to:1) perform an overload expert opinion of the ceiling in the room where the tomograph will be installed and submit the original to the Ordering Party.22) Provide a CT scanner with the parameters specified in the Terms of Reference;3) Perform adaptation works in the rooms, provide, set up and start up the subject of the order at the location indicated by the Ordering Party, using the necessary materials, parts and equipment;4) In case of building reconstruction - obtain a building permit;5) Initiate actions and prepare, at their own expense, all documentation that the Ordering Party, as a party to the proceedings, will submit to appropriate authorities in order to obtain approvals necessary for the proper and legal use of the device.66) perform acceptance and specialist tests during the warranty period; 7) verify the correct operation of the equipment with the participation of the Ordering Party;8) train the Ordering Party's personnel in the use of the tomograph (min. 5 people);9) provide a warranty and deliver documents confirming it;6.
