Description

The purpose of this document is to provide interested parties with information to enable them to prepare and submit a proposal for services for the Connections Count Prevention/Intervention Program (Connections Count) – Milwaukee County to provide an outreach and resource referral program aimed at vulnerable young families. Connections Count is an initiative targeting vulnerable families with children under the age of 18 that reside in Milwaukee County.

The intent of the Connection Count initiative is two-fold:
1. To improve the connectedness of vulnerable families with young children to ongoing formal services, community-based resources, and natural supports; and,
2. To assist these families in accessing benefits and services that could meet their needs.

By connecting vulnerable families with children to needed formal services, resources, and supports, Connections Count will help children continue to be cared for in their own homes and avoid costly hospitalization, emergency department visits, and potential involvement in the child welfare system. Current research on savings and cost-effectiveness of similar initiatives shows promising results.

DCF intends to use the results of this Request for Grant Proposal (RFP) to award a minimum of one contract. Non-profit organizations, for-profit organizations, local units of government and tribal governments are eligible to submit a response to this RFP.
