Description

Description of the subject of the contract (OPZ) - Attachment No. 2 to the ToR1. The subject of the contract is to provide a comprehensive service consisting in cleaning hospital buildings, including medical and non-medical areas, patient assistance and medical and non-medical in-hospital transport. A detailed description of the subject of the contract is contained in appendices no. 1-5.3 to OPZ.2. The Contracting Authority recommends a site visit (appointment for a site visit - contact with Beata Goszczyńska Tatarczuk, tel. +48 666852442, P. Małgorzata Rydel-Pernak, tel. +48 664032044).3. The Contractor will provide the Coordinator of the facility and two shift foremen throughout the contract period, in accordance with appendix no. 4 to OPZ, and two shift foremen working in the system.4.The contractor shall ensure a minimum daily manning of personnel cleaning medical areas in accordance with Appendix 4 to OPZ.5.The Contractor shall ensure the minimum daily manning of the transport team in accordance with Appendix 4 to OPZ.6. The Contractor shall ensure the minimum daily manning of persons for machine washing, acrylic cleaning, maintenance and polishing of floors in accordance with Appendix 4 to OPZ.7. The Contractor shall ensure the minimum daily manning of personnel cleaning non-medical areas in accordance with Appendix 4 to OPZ.8. Before signing the contract, the Contractor shall submit to the Contracting Authority the list of staffing numbers in individual facilities, which after approval by the Contracting Authority shall constitute an Appendix to the contract. The employee is a person employed on the basis of an employment relationship or civil-law contract.9. The list of current location of organizational units in particular pavilions of the Wolski Hospital is specified in appendix no. 6 to OPZ.10. The list of areas together with the list of rooms of the Wolski Hospital is specified in appendix no. 7 to OPZ.11. The list of assortment needs used for cleaning and transport services is specified in appendix no. 8 to OPZ.12. The list of window areas is specified in appendix no. 9 to OPZ.13.The list of washbasins, sanitary facilities, bathrooms is specified in appendix no. 10 to the Collective Labour Agreement 14. The Contractor shall be obliged to conclude a loan agreement for 4 subscription numbers in the cellular network applicable in the hospital.15. The Contractor shall be obliged to conclude a loan agreement for 115 chip cards including electronic code locks in Pavilion 8 - SOR area, Pavilion 5 and Pavilion 9.16. The Contracting Authority shall rent the premises designated for the performance of the agreement (list of premises is specified in appendix no. 9 to the ToR). Detailed rules of lease of premises have been specified in the draft lease agreement, which is appendix no. 11 to the ToR.17. The list containing information on the areas to be leased, including data necessary for valuation of services being the subject of this proceeding is appendix no. 10 and appendices no. 6 to 10 to the ToR.
