3.1 The subject matter of the contract is the successive supply of disinfectant products to the place indicated by the Purchaser. A detailed list of assortment is included in Appendix No. 2 - Price Form attached to the SWZ, Code according to CPV: 33631600-8 - Antiseptics and disinfectants3.2 The subject matter of the order is to be new, authorized for marketing on the basis of applicable laws and comply with all requirements set forth by law, in particular the Law of April 7, 2022 on medical devices (Journal of Laws of 2022, item 974), and free from any physical or legal defects and have an expiration date of not less than 12 months on the date of delivery.3..3 The order is divided into 16 parts:Set No. 1 - hand washing and disinfectionSet No. 2- disinfection of equipment and surfacesSet No. 3 - washing and disinfection of skinSet No. 4 - disinfection of instrumentsSet No. 5 - disinfection of equipment and surfacesSet No. 6 - disinfection of skin and mucous membranesSet No. 7 - washing of skin and mucous membranes.

Within a part, all listed items must be offered. Failure to fill in any of the items within the part will result in rejection of the offer.3.4. It is required that each individual item of the offered product is provided with a commercial label made in Polish, the same label should be provided with the bulk packaging. The commercial label should contain at least the following information:3.4.1.type and name of the product 3.4.2.name of the manufacturer 3.4.3 batch number 3.4.4 expiration date3.5. If the commercial name of a company, commodity or product is indicated in the tender documentation, the Contracting Authority shall allow equivalent solutions in accordance with the technical data and parameters contained in the documentation. If the Contracting Authority allows equivalent solutions to those described in the documentation attached to the SWZ, but does not specify the minimum parameters that would confirm this equivalence - the Contractor is obliged to offer a product with similar properties, functionally suitable for the required application (arg. based on the sentence of the judgment of the National Board of Appeal dated October 14, 2013. [ref: KIO 2315/13]). A contractor who refers to equivalent solutions described by the Contracting Authority is obliged to demonstrate that the supplies offered by him meet the requirements specified by the Contracting Authority. NOTE: The proper name is the name under which the item designated by it appears (or would appear) both in Poland and in other countries.3.6 Whenever trademarks, patents, origin, standards, approvals, technical specifications and technical reference systems are indicated in the description of the subject of the contract, the Contracting Authority shall allow equivalent solutions.
